What is a Treatment Summary + Care Plan?

This is where our Oncology Nurse steps in

When a survivor is first diagnosed with cancer, they’re often flooded with information. It’s usually only after treatment ends that they have the time and space to process everything they’ve been through. This is where Pam steps in—providing re-education, offering reassurance, and always guiding them to their physician when needed. She also encourages survivors to focus on caring for their overall well-being, not just the aspects related to their diagnosis, by connecting them with the resources and activities available at the Cancer Survivorship Center.

Your Treatment Summary

Organizes essential information about your cancer care history. This includes diagnosis details, treatments you received (such as surgeries, chemotherapy, radiation therapy) and any additional therapies. This information comes directly from your medical record.

Survivorship Care Plan

Outlines your recommended follow-up care. This includes how often you should have follow-up visits, what tests or screenings are recommended, and potential long-term or late effects of treatment. It also provides guidance for healthy living moving forward.
